AFFIDAVIT

I, Lakhmi Chand son of Shri Hari Chand, Resident of _________________________, do hereby solemnly affirm and declare as under :-

1.               That I am holder of a Ration Card bearing Ration Card No. ___________. 

2.             That on ______________the above said Ration Card has been lost/misplaced by me in the market of NH-2, NIT, ____________, and could not be traced after due effort. 

3.               That if I found the same then I will not misuse the same and informed the concerned authority.

4.               That I want to lodge report against the lost of my Ration Card.
