AFFIDAVIT

I, Dharmendra Rawat @ Dharmendra Kumar, R/o _______________________, do hereby solemnly affirm and declare as under :-

1.   That I am permanent resident of the above said address.

2.   That my name is written on some of documents is Dharmendra Kumar.

3.    That myself known as Dharmendra Rawat in my society.

4.   That Dharmendra Kumar and Dharmendra Rawat are same person.

5.   That I want to change my name as Dharmendra Rawat instead of Dharmendra Kumar and myself should be known as Dharmendra Rawat in future.

6.   That in all the records the name of myself may kindly be recorded as Dharmendra Rawat instead of Dharmendra Kumar.
